So here’s my story, it’s kind of long. I found out I was pregnant in February of 2009. My husband and I were a little surprised by the pregnancy, well very surprised, but happy for the new baby on the way. The pregnancy went by just fine. Finally October came and the baby came a week after the due date. The labor was long, about 30 hours but I was able to deliver him vaginally after being told they scheduled me for a C-section.
We had a baby boy, but because he had meconium he was sent to the Neonatal Intensive Care Unit. After I had him I was having some discharge that I was told was the left over meconium, but a week went by and I was still having the discharge. I realized I had a tear that was more serious then the 1st degree tear that was repaired after the birth and that the nurse midwife had missed the tear at delivery.
1st Repair - failed
The Ob had me come in immediately, did an examination and found that I did indeed have a 4th degree tear and the midwife at the birth had missed the tear. I was scheduled for surgery within the next week. Unfortunately, the surgery only held for two days and when I called the Ob office to tell them it didn’t work they referred me to a colon-rectal specialist.
2nd Repair - failed
The new doctor did another examination and found the tear had re-opened and created a fistula. He recommended another surgery in 6 weeks. So after the New Year I went in for surgery again and this time the surgery lasted a whole week, but yet again it failed.
I tried to search for answers from the colon-rectal doctor, but he did not want to give me any real information. Frustrated with the treatment I was receiving I wanted to find another doctor. I caught a huge break when I found out about Dr. Miklos. At the time I was an intern at a chiropractor’s office in Tucker working to finish up my doctorate in chiropractic. One of the women who worked for the chiropractor also had had surgeries in the past that had failed, not for the same condition, but we chatted about our experiences. After sharing my story with her she told me about her daughter who worked for a Urogynecologist (which happened to be Dr. Miklos).
3rd Repair - cured
Lucky for me he could perform the surgery and not only that, he is one of the top rated surgeons in the world. I met with Dr. Miklos and to my surprise he was very informative, sat down with me and actually talked to me about what he would do and possible outcomes. He was very realistic yet reassuring which made me feel very comfortable. So into surgery I went for a third time, praying that this one would work! "July 2008, I developed a vescovaginal fistula 3 weeks after I was recovering from a complicated total abdominal hysterectomy with a severe case of endometriosis. On December 2008 my Urogynecologist first attempted to repair it through the vaginal approach, shortly after new years I had leakage from the vaginal area. Three months later I was given an alternative of proceeding with a subsequent repair through the abdominal route which a placement of an omental flap versus a repeated attempt through the vaginal route. I refused the abdominal route and went ahead to re-attempt the vaginal route again in March of 2009. After the March surgery a cystogram revealed no leakage or signs of a fistula and catheters were removed. Two weeks after, I developed another recurrence and another cystogram confirmed there was definitely a leakage in the vaginal area. My urogynecologist discussed again about having it repaired via abdominal route. I strongly wanted to have it repair laparoscopically, but the adhesions were a concern for my urogynecologist. I also did not want another laparatomy due to the experience of having a difficult time recovering from the total abdominal hysterectomy surgery.
I searched online and found Dr. Miklos. On October 2009 I emailed Dr. Miklos my questions and concerns, it was worth a try. I was very surprise to receive a quick response from him. Dr. Miklos was very helpful in answering and reading my medical notes even if I wasn’t a patient of his at that time. While I was emailing Dr. Miklos, my Urogynecologist in Hawaii offered to try a less invasive surgical procedure that would involve trying to repair the fistula with glue. It was worth a try before flying out to Atlanta. It was a chance I was taking and was hoping it would work. November 2009 I once again attempted to try it via vaginal route with glue, which at the end of the week the fistula recurred and I was faced with another disappointment.
With all 3 failed surgeries, I asked my Urogynecologist if she has heard about Dr. Miklos, which she did. I mentioned to her that I wanted to see him because he is known to conduct a repair laparoscopically. My Urogynecologist was very supportive with me seeing Dr. Miklos and felt that having the surgery completed successfully laparoscopically would be greater with Dr. Miklos. I contacted Rhonda at Dr. Miklos office and scheduled my appointment for March 2010. Rhonda was great and answered all of my questions I had, she was very easy to talk too and very supportive. I was very nervous traveling thousands of miles to have surgery done in a state that I’ve never been too, but when I got there the staff was very friendly I felt like I was at home in Hawaii. His staff arranged a hotel that was really close to Northside hospital which was very convenient for my husband & kids.
When I met Dr. Miklos, I immediately felt comfortable with him as he explained the procedure in detail and answered any questions my husband and I had. I felt he was very confident in his work, which made me even more comfortable and trusted him to perform the vesicovaginal repair. Today, I’m not dealing with anymore urinary pads, don’t have to worry about having an accident, and I’m able to do my normal activities. (Translated from Spanish below) I gave birth in January 2010 and suffered a 4th degree tear that was not properly repaired. Ten days later I went to the hospital after having complications. I was told there was a defect and that it needed to be repaired right away. It was repaired again and I went home. Turns out it was too soon after the initial repair to have that done, my tissue was too swollen. Three days later, I was devastated after an experience using the restroom and went back to the hospital and learned I had a rectovaginal fistula. It was such a horrific and depressing experience. It was a time when my husband and I were supposed to be full of joy, celebrating the birth of our wonderful son. I felt as though there was no hope and did not want another surgery.
My husband and I started doing research and were determined to find the best doctors no matter where we had to go. We learned of Dr. Moore and Dr. Miklos after doing research on extensive fistula repair. We learned they had actually performed skin grafts on some patients during fistula repairs at Brigham Women's Hospital in Boston, (a teaching affiliate of Harvard Medical School). Tuve a mi bebe en Enero del 2010 y sufri un desgarro nivel 4 que no fue propiamente reparado. Despues de 10 dias tuve complicaciones y volvi al hospital donde me dieron a conocer de que habia un problema y necesitaba ser sometida a una cirugia. I had an open abdominal hysterectomy in January 2010. A few days later, I found I was always leaking and never dried. Upon CT scan in May, I was confirmed to have a vesicovaginal fistula. To my horror, all doctors I saw in Hong Kong suggested another open abdominal surgery for repair. I refused and searched on the internet for doctors who might be able to offer a less invasive approach.
The internet led me to Atlanta Urogynecology Associates. The laparoscopic operation videoed by Dr. Miklos brought me great interest. I contacted AUA and asked several questions on how my disease could be cured. Dr. Moore replied me with 5 miles of details on the laparoscopic procedure, how it would be done, how many small openings I will have, where the openings are, what hospital, how long I need to be in area, why laparoscopic is better than open surgery, and the after operation care, etc. He gave me great confidence that he is the doctor for me! Thus I confirmed the surgery with him. His assistants were also professional and helpful in the follow up. They advised me what test I needed to do and take to AUA. I was worry-free as they helped arrange the hospital and hotel.
From Hong Kong to Atlanta, the laparoscopic surgery took place in August. The operation went smoothly. There was no complication, no pain. It was as exactly as Dr. Moore described prior to the surgery. I was able to walk around the Perimeter Mall 3 days after the surgery. I was nervous scheduling surgery with a Doctor I hadn't met in fact I found Dr. Miklos through his website….. but after 2 failed surgeries in N.Y. I know I had to travel to find a Dr. that could help me. I needed a vescovaginal fistula repair, the doctors here wanted to go back in but I wanted a laproscopic procedure. I have referred 2 patients this year to Drs. Moore and Miklos and I am extremely pleased with their professional care and follow up. One patient experienced a vesicovaginal fistula after her hysterectomy followed by a failed vaginal repair by a Urologist and myself. Rather than repair the recurrent vaginal apical fistula with open technique, I referred her to Dr. Moore to do a Laparoscopic Repair of Vesicovaginal Fistula at the apex of the vagina. The patient got excellent results with minimally invasive techniques developed by Drs. Moore and Miklos with only a short time in the hospital and short interval to return to work. Their operative skills and techniques are definitely superior to the ones typically used by the Urologist or general Gynecologist. This, in turn, benefits the patient in care, recovery and satisfaction. The time required to develop these specialty skills and techniques requires a large number patients with similar problems which fortunately, the general Gynecologist does not have. I also was allowed to observe the entire procedure which was educational in itself.
I would urge Gynecologists faced with failed or primary vaginal support problems, urinary incontinence problems, or post-op vesicovaginal fistulas, to consider referral for evaluation since I do believe Drs. This patient sustained a VVF after getting a hysterectomy for Ovarian Cancer. After the operation she had a continuous urine leakage from her vagina. She went to her Urologist who recommended a large incision (laparotomy) on her belly. The patient asked about a laparoscopic approach to the repair and going to see Dr Miklos & Moore and the urologist got ANGRY and basically dismissed her from the practice.
